Please provide the infomation listed below to your assignment manager and to nop@ndu.edu. The NATO Orientation Program must receive this information to hold a quota for you. Please confirm your enrollment by e-mail 14-21 days before the start of class.

• Class Dates Desired
• Name with Suffix, Prefix
• Rank/Title
• Gender
• Ethnicity
• Date of Birth
• Branch of Service (military only)
• Specialty (military only)
• Current Agency/Unit Name (please spell out)
• Unit Address and Phone
• Current or Future NATO-Related Duties** (please spell out command name/location/nation)
• Arrival Date to Gaining Command
• Citizenship
• E-mail Address and Phone Number (please ensure we can reach you while you are enroute)
• Previous NATO Experience

**Please include sufficient justification for your course attendance if your current or future duties are not NATO-related or in a NATO nation.
